DoIT Computer Lending Program Request

This form creates a ticket for the DoIT Computer Lending Program which is managed by the Walk-In Help Desk. Individual reservation requests are not permitted, and bulk reservation requests are subject to approval. Equipment is served on a first-come, first-serve basis at the Walk-In Help Desk, and availability is not guaranteed. Departments are highly encouraged to work with local IT on available resources before reaching out to DoIT.
